Katara's biggest mistake was letting Azula know.

It wasn't as if she had done it on purpose. Actually, she still wasn't sure exactly how the former princess (now a relatively minor government official; she'd managed to keep her place at the palace through the end of the war, but she'd lost just about everything else) had worked it out. They had just been having a particularly heated argument on night, and Azula had gotten right into her face, and then suddenly she pulled back, silently mouthing an 'Oh' of realization. And then Katara felt a sinking feeling in her stomach as she realized that Azula now knew.

Ever since then, whenever they'd been forced to work together, Azula had stood next to her, got into her personal space, casually touched her on the shoulder...she even went so far as to have a whole new wardrobe commissioned, featuring startlingly low-cut and revealing robes and other outfits, which she wore only when dealing with the ambassador from the Southern Water Tribe. Anything to get under Katara's skin.

There was nothing illegal or even improper about it, but Katara knew that Azula was doing it deliberately. They might not technically be enemies anymore, but that didn't mean they liked each other. And Azula liked to go out of her way to make even her staunchest allies uncomfortable.
